A somewhat simple, somewhat unorthodox guide to the best erections in your life!

What makes the perfect erection? Here's a little guide to getting the best erections possible, regardless of your age.

1. Perfect penile hydraulics

You will need:

1. Optimal blood pressure/arterial pressure
2. Maximum vasodilation of the penile arteries

Blood pressure is a tricky marker for erectile dysfunction, despite what doctors want you to believe. If you think about it, the only way a car tyre can become pumped and hard is when the pressure inside is high. Your cock requires that element of pressure to become rock-hard and skin bursting.

Too low of a blood pressure (excluding endurance athletics) and your cock cannot pump itself up. The symptoms of low blood pressure includes cold extremities, lightheartedness and even constipation. This indicates poor blood flow to the affected areas, which will also include your penis.

But don't most Singaporeans have high blood pressure, have the above symptoms, and gasp, weak erections as well? Well, most of these patients are both sedentary and eat a diet that encourages arteriosclerosis. Short term solutions:

x Yohimbine (illegal to sell and import) to increase arterial pressure
x Medical grade penis pumps to force blood into the corporal cavernosum

Long term solutions:

- Cardiovascular exercise to increase cardiac output
- Dietary changes to reduce arteriosclerosis (clear the plagues in your arteries)
- Investigate and fix possible venous leakages or mechanical arterial dysfunctions via surgery
- Increase electrolyte intake (yes, this includes sodium) to increase arterial pressure

2. Perfect balance of neurotransmitters and chemicals in your penis

Yes, your penis is a muscle! It is made of smooth muscle just like your heart and other internal organs. It has many circulating neurotransmitters and chemicals that can affect its ability to expand (allow blood in) and contract (blood flow out).

One compound is Nitric oxide (NO2). This is responsible for vasodilation (expansion) of smooth muscle, allow your penis to get even more engorged. This is while nitrogylcerin is given to heart attack patients. Viagra is used similarly for the penile smooth muscle.

Dihydrotestosterone (DHT) also very important for health penile smooth muscle. In fact, this is the same hormone that is (mostly) responsible for penis size and growth! DHT is also given topically to FTM patients and micropenis cases to try to grow a bigger penis. Rats studies have proven how important DHT is to erectile function, size and preservation.

DHT is formed from Testosterone which is why men with low T levels are associated with erectile dysfunction. But while testosterone levels matters for libido (sexual desire), it is limited in the actual erectile function. High T means high DHT. Also, this why some people on Testosterone replacement therapy still continue to have ED, because testosterone is not the end all.

Hence, we want to consider factors that can limit DHT. This includes adipose tissue (body fat) . This body fat (not dietary fat, mind you!) blunts the potency of DHT by chemically breaking it down into a watered-down compound. It also aromatises testosterone into estrogen, indirectly reducing the amount of DHT produced.

...and it also increase oxidative stress that increases Cortisol levels. Cortisol is known as the stress hormone in the body. It then causes Norepinephrine levels to rise. Norepinephrine, remember this word, for this is literally the 'stress' hormone for your cock. When you have circulating norepinephrine in your penis, it leads to vasoconstriction (preventing blood flow in) which prevents you from getting a full erection. Also, it is believed that general lifestyle 'stress' can increase norepinephrine. This is also why doctors believe that erectile dysfunction can be "all in the mind". Well it isn't that straightforward. Anxiety disorders ("including performance anxiety") and chronic stress can to a certain extend increase norepinephrine levels, which in return causes the contraction of your penile muscles.

Other neurotransmitters include Dopamine and Seratonin, which to be frank are more complex when it comes to erectile function (which is why patients with depression who happen to also have erectile dysfunction are particularly tricky to treat). In general, increasing dopamine leads to increase levels of Nitric oxide, and a decrease in norepinephrine output. Lastly, tumours and endocrine dieases can also cause ED by the same processes.

Short term solutions:

- PDE5s (viagra, cialis) to increase specific penile vasodilation
- High dose NO2 supplements to increase systematic vasodilation (limited effect)
- Penile injections to increase specific penile vasodilation
x Uroselective Alpha-1 blockers (tamsulosin, alfuzosin) to reduce norepinephrine in the penis
x Alpha-2 blockers (yohimbine, again illegal) to increase nitric oxide output
x Nitroglycerin to increase systemic vasodilation
x Low dose anxiolytics that can blunt norepinephrine

Long term solutions:

- Losing excessive body fat to increase circulating DHT
- Ensure testosterone levels are optimal and maximal via hormonal therapy
- Reduce as much lifestyle stress as possible
- Enjoy the sex and don't feel pressured!
- Avoid medication such as Accutane (for acne) and Finasteride (for hair loss) that are essentially DHT blockers
x Herbs and supplements that might increase testosterone or increase vasodilation
x Daily, low dose, long term use of long acting PDE5s (cialis) to ensure optimal smooth muscle state at all times
x Low dose anxiolytic


3. Nerve function

This section is pretty straight forward, for you want the nerve pathway connecting your brain and penis to be healthy. If you suffer a spinal cord injury, its pretty much good bye to your penis. If you have prostate surgery and they do not spare the nerve, erectile function and sensation is also loss, if not greatly diminished. Lastly, neurodegenerative diseases which also includes diabetes can also damage these precious nerve endings.

But, nerve cells in general tend to follow the brain cell model..Use it or lose it! Just like how route learning works, the nerves responsible for your erectile function behave the same-way. Theoretically. That's why doctors recommend patients with ED, or even post prostate surgery patients to continue having sex as much as possible.

Long term solutions:

- Have sex regularly
- Drive safe so you do not end up a vegetable with a limp dick
- Supplements for nerve health (limited effect)
x Nightly low dose viagra (25mg-50mg) to ensure strong, nocturnal erections regularly


4. Structural integrity of the pelvic floor

While the penis itself is made of smooth muscle and cannot be subjected to conscious tension and release, the muscles wrapping around your penis and entire pelvic floor are skeletal muscle. So when you "gek" your cock to make it bounce, you are actually contracting the muscles at the base of your penis, not the penis itself.

Dad posture, tight muscles and a weak core (e.g the average desk worker) can lead to tightness and dysfunction in your pelvic floor. This can and will affect your erectile function. Just like how tight calves and hamstrings can reduce performance for runners. Another cause is trauma to the pelvic floor and the surrounding muscles.

A less likely cause would be pelvic floor weakness. Mainstream medical media seem to promote kegels for men to "strengthen" their erections. The fact is, it is unlikely for muscles in general to become weak and waste away on their own, unless there is some form of trauma or neurological disease. You use your pelvic muscles almost 24/7. If they were actually weak, you would be peeing your pants and shitting yourself. So I recommend visiting a physio specialized in the pelvic floor to see what is the real problem. Can you imagine having a frozen shoulder and trying to do shoulder extension exercises to make it work better? Sure sounds silly, and that is the same with kegels.

Short term solution:

x Muscle relaxants to relief pelvic floor tension
x Uroselective Alpha-2 blockers to relief pelvic floor tension

Long term solution:

- Visit physiotherapist to solve pelvic floor dysfunction
- Visit physiotherapist to solve pelvic floor weakness
x Botox shots into the pelvic floor to relax it


I hope this guide can help most guys with weak erections or ED via process of elimination. This was my own process of elimination and it worked for me. Or for hardcore guys who want to experiment in order to get amazing erections. Feel free to ask any questions.
